Скомпилировать как отладочную версию, запустить в IDE. Он покажет вам, где произошла ошибка.

(msvcr90.dll говорит мне, что вы используете релизную сборку).

Просто интересно, правильно ли вы включили библиотеку и каталоги в Visual Studio? У rastertek есть хорошее пошаговое руководство на случай, если что-то пойдет не так.
